Each clinical vignette is held to five things together: evidence, a current guideline when one applies, realistic clinical detail, the reasoning that exam asks for, and a patient-safety point. A case is not written from a question template alone. The step has to be traceable to a source that was opened, then shaped into a fictional patient.

A traceable item has a checked guideline link and a short note of how that source applies. Traceable is not physician-reviewed, and it is not published. If no strong guideline applies, the section is omitted rather than filled with a guessed link.
